Less is known about the impact of Paternal Smoking. Methods: We estimated differences in semen parameters and testicle size according to Paternal Smoking in 772 adult sons of women enrolled in the Danish National Birth Cohort when pregnant. Parents’ Smoking was reported around gestational week 16, and analyses were adjusted for parents’ ages at conception, maternal pre-pregnancy body mass index, maternal alcohol and caffeine intake, family occupational status, ejaculatory abstinence time, clinic of semen analysis, and season. Results: Sons of Smoking fathers and non-Smoking mothers had a 10% (95% confidence interval: −24%, 7%) lower semen concentration and 11% (95% confidence interval: −27%, 8%) lower sperm count than sons of non-Smoking parents. Having two Smoking parents was associated with 19% reduction in sperm count (95% confidence interval: −37%, 3%). Paternal Smoking was not associated with volume, motility, or morphology. Adjusting for maternal Smoking, Paternal Smoking was associated with a 26% increased risk of small testicular volume (95% confidence interval: 0.89, 1.78). Discussion: Exclusion of sons with a history of testicular cancer, chemotherapy, orchiectomy, and with only one or no testicles may have caused us to underestimate associations if these men's reproductive health including semen quality are in fact more sensitive to Paternal Smoking. Conclusion: The study provides limited support for slightly lower sperm concentration and total sperm concentration in sons of Smoking fathers, but findings are also compatible with no association. Nevertheless, our team recently detected a lower total sperm count in the sons of Smoking fathers as compared to sons of non-Smoking fathers. Since Paternal and maternal tobacco Smoking often coincide, it is difficult to discriminate whether effects are mediated Paternally or maternally when using questionnaire- or register-based studies. Therefore, getting an objective measure of the maternal nicotine exposure level during pregnancy might help disentangling the impact of Paternally and maternally derived exposure. OBJECTIVES: Our aim was to study how Paternal Smoking at the time of the pregnancy was associated with semen quality in the sons after adjusting for the maternal levels of nicotine exposure during pregnancy. METHODS: We recruited 104 men (17-20 years old) from the general Swedish population. The participants answered a questionnaire about Paternal Smoking. Associations between Smoking and semen volume, total sperm count, sperm concentration, morphology and motility were adjusted for levels of the nicotine metabolite cotinine in stored maternal serum samples obtained from rubella screening between the 6th and 35th week of pregnancy. We additionally adjusted for the estimated socioeconomic status. RESULTS: After adjusting for the maternal cotinine, the men of Smoking fathers had 41% lower sperm concentration and 51% lower total sperm count than the men of non-Smoking fathers (p = 0.02 and 0.003, respectively). This was robust to the additional adjustment. CONCLUSIONS: Our results suggest a negative association between Paternal Smoking and sperm counts in the sons, independent of the level maternal nicotine exposure during the pregnancy. (Less)

Nicotine concentrations in hair were measured using liquid chromatography–tandem mass spectrometry to determine long-term exposure to SHS. Results Differences between the smoker group and nonsmoker group in nicotine levels were statistically significant in both children and their mothers. However, difference between the indoor-smoker group and outdoor-smoker group was marginally significant in children and was not significant in their mothers. In the indoor-smoker group, preschool children and their mothers had nicotine concentrations about twice as high as school children and their mothers, respectively. In the outdoor-smoker group, however, differences between two age groups in nicotine levels were significant in preschool children, but not their mothers. Conclusion These findings indicate that Paternal Smoking at home leads to significant exposure to SHS in their children and spouses, which is not completely prevented by Smoking outside. Especially, preschool children and their mothers appear to be most at risk for SHS exposure among nonSmoking household members.

The authors investigated fetal exposure to Paternal Smoking at home during pregnancy. Korean families were included as trios of fathers, mothers, and neonates identified in 2005-2007. Sixty-three trios were finally enrolled in this study after exclusion of those in which the mother was a smoker or was regularly exposed to ETS at places other than the home. Nicotine and cotinine concentrations in hair were measured by using liquid chromatography-tandem mass spectrometry to determine long-term exposure to ETS. The difference between neonatal nicotine concentrations in the smoker and nonsmoker groups was not statistically significant. However, in the indoor-smoker group, neonatal nicotine concentrations were significantly higher than in the outdoor and nonsmoker groups (P < 0.05). Furthermore, neonatal nicotine concentrations in the outdoor-smoker group were not different from those in the nonsmoker group. These findings indicate that Paternal Smoking inside the home leads to significant fetal and maternal exposure to ETS and may subsequently affect fetal health. Conversely, findings show that Paternal Smoking outside the home prevents the mother and her fetus from being exposed to ETS.

The prevalence of Paternal Smoking was 72.9%. In families with and without a father who smoked, the mean (standard error) household food insecurity score was 1.83 (0.01) and 1.48 (0.02), respectively (p<0.0001). Paternal Smoking was associated with greater household food insecurity score (p<0.0001) in a multivariable linear regression model, adjusting for maternal age, maternal education, weekly household expenditure per capita, and province. Among poor families in rural Indonesia, households with a father who smokes are at greater risk of household food insecurity.

Paternal Smoking diverts money from basic necessities to cigarettes and adversely affects child health; tobacco control should therefore be considered among strategies to improve child survival.

Main outcome measures were child underweight (weight-forage Z score ,22) and stunting (height-for-age Z score ,22) and severe underweight and severe stunting, defined by respective Z scores ,23, for children aged 0– 59 months of age. Results: The prevalence of Paternal Smoking was 73.7%. The prevalence of underweight and stunting was 29.4% and 31.4%, and of severe underweight and severe stunting was 5.2%, and 9.1%, respectively. After adjusting for child gender, child age, maternal age, maternal education, weekly per capita household expenditure and province, Paternal Smoking was associated with an increased risk of underweight (odds ratio (OR) 1.03, 95% confidence interval (CI) 1.01 to 1.05, p = 0.001) and stunting (OR 1.11, 95% CI 1.09 to 1.13, p,0.001) and severe underweight (OR 1.06, 95% CI 1.01 to 1.10) p = 0.020) and severe stunting (OR 1.12, 95% CI 1.08 to 1.16, p,0.001). Conclusions: Paternal Smoking is associated with an increased risk of child malnutrition in families living in rural Indonesia.

Ricks, Martin W BloemAbstract:Objective: Paternal Smoking is highly prevalent in Asia, and tobacco may account for a large proportion of household expenditures among poor families. We sought to characterise the relationship between Paternal Smoking, child malnutrition and food expenditures. Design: Data on Smoking, household expenditures and child malnutrition were examined in a stratified multistage cluster sample of households in the Indonesia nutrition surveillance system. Main outcome measures were child wasting (weightfor-height Z-score ,22), underweight (weight-for-age Z-score ,22) and stunting (height-for-age Z-score ,22), and severe wasting, underweight and stunting (defined by respective Z-scores ,23). Setting: In total, 175583 households from urban slum areas in Indonesia. Subjects: Children 0‐59 months of age. Results: The prevalence of Paternal Smoking was 73.8%. After adjusting for child gender and age, maternal age and education, and weekly per capita household expenditures, Paternal Smoking was associated with child stunting (odds ratio (OR) ¼ 1.11, 95% confidence interval (CI) 1.08‐1.14, P , 0.0001), severe wasting (OR ¼ 1.17, 95% CI 1.03‐1.33, P ¼ 0.018) and severe stunting (OR ¼ 1.09, 95% CI 1.04‐1.15, P , 0.001). In households where the father was a smoker, tobacco accounted for 22% of weekly per capita household expenditures, with less money spent on food compared with households in which the father was a non-smoker. Conclusions: Among poor families in urban slum areas of Indonesia, Paternal Smoking diverts household money from food to tobacco and exacerbates child malnutrition.

Paternal Smoking and Pregnancy Loss: A Prospective Study Using a Biomarker of Pregnancy

The authors examined the effect of Paternal Smoking on the risk of pregnancy loss in a prospective cohort of 526 newly married, nonSmoking, female textile workers in China between 1996 and 1998. Upon stopping contraception, subjects provided daily urine specimens and records of vaginal bleeding for up to 1 year or until clinical pregnancy. Daily urinary human chorionic gonadotropin was assayed to detect conception and early pregnancy losses, and pregnancies were followed to detect clinical spontaneous abortions. Subjects were grouped by the number of cigarettes that husbands reported Smoking daily: nonsmokers (group 1, n = 216), fewer than 20 cigarettes (group 2, n = 239), and 20 or more cigarettes (group 3, n = 71). Compared with that for group 1, the adjusted odds ratio of early pregnancy loss of any conception for group 2 was 1.04 (95% confidence interval (CI): 0.67, 1.63) and for group 3 was 1.81 (95% CI: 1.00, 3.29). The adjusted hazard ratio of conception for group 2 was 0.90 (95% CI: 0.70, 1.18) and for group 3 was 0.96 (95% CI: 0.66, 1.39), while the adjusted hazard ratio of clinical pregnancy for group 2 was 0.93 (95% CI: 0.72, 1.20) and for group 3 was 0.78 (95% CI: 0.55, 1.12). Our analysis included 1,718 children 8 to 15 yr of age whose mothers were never-smokers. Multiple linear regression models were used to estimate the effect of Paternal Smoking on children's pulmonary function, with adjustment for children's age, sex, weight, height, square of height, asthma, and father's education. When compared with children of never-Smoking fathers, children of Smoking fathers had small, but detectable deficits in FEV1 ( − 36 ml, SE = 20) and FVC ( − 37 ml, SE = 22). When children of Smoking fathers were subdivided into two subgroups, father smoked < 30 cigarettes/day and ⩾ 30 cigarettes/day, we found that children whose fathers smoked ⩾ 30 cigarettes/day had the largest deficits in both FEV1 ( − 79 ml, SE = 30) and FVC ( − 71 ml, SE = 34).
